Vashon Senior Center seeking Geriatric Social Worker

Vashon Senior Center is seeking a qualified Geriatric Social Worker to provide part-time social work services to assist older adults, their families and caregivers in accessing healthcare, social services, and community resources. Responsibilities include:

  • Conducting needs assessments for vulnerable older adults.

  • Coordinating care and connecting older adults to healthcare and social service providers.

  • Providing guidance on Medicare, Medicaid, housing assistance, and food programs.

  • Supporting patients with dementia and connecting them to dementia resources.

  • Supporting seniors facing life transitions and end of life planning.

  • Conducting outreach and education for seniors, families, and caregivers.

  • Advocating for seniors' healthcare and social service needs.

  • Helping to build and maintain partnerships with local healthcare providers and community organizations to support the older adult community via participation in Vashon Social Service Network and other avenues.

Flexible Schedule. Goals include a minimum of 1 to 2 days of in-person support at Vashon Senior Center, availability for phone support and in-home visits, plus outreach provided throughout the island community. Funding from Vashon Health Care District will support up to 80 hours per month of social work support. Desired minimum commitment is 15 hours per week (60 hours per month). Qualifications:

  • BSW with a concentration in gerontology and at least 5 years of experience.

  • Preferred MSW with experience in gerontology.

  • Strong communication skills and ability to liaise with healthcare providers, older adults, and families.

  • Expertise in healthcare services, Medicare, and Medicaid.

  • Knowledge of Vashon and Puget Sound area resources.

  • Experience navigating in a small rural community with dual relationships

  • Experience working independently.

  • Commitment to a good working practice of patient confidentiality and compliance with HIPAA where appropriate. Knowledge of National Association of Social Worker Code of Ethics and maintenance of those standards in their practice.

Position will be an Independent Contractor contracted and paid by Vashon Senior Center with funding support from Vashon Health Care District.
